Diabetes Prevention Program
Originating University: University of Arizona
A series of one-page educational supplements to be integrated into the CDC’s National Diabetes Prevention Program (DPP) Prevent T2 Facilitator Guide. These materials are designed for use by lifestyle coaches during select group sessions. Each supplement aligns with the topic of a specific DPP session and introduces research-based, conversation-ready immunization talking points that connect to the session’s core content.
If you choose to adopt an EXCITE program, we encourage you to connect with your regional EXCITE Health Agent to support the implementation, reporting, and evaluation process so we can collectively demonstrate program impact to stakeholders. Find your regional health agent contact here.

- Participants will understand how health-promoting behaviors are linked with a stronger immune system and decreased risk of communicable diseases and build confidence in using research-based resources to make health decisions that align with their personal values.
- Participants will engage in group conversations that connect immunizations with other health-promoting behaviors, contributing to a supportive environment for exploring questions, addressing hesitancy, and sharing personal experiences.
