About: A series of one-page educational inserts to be integrated into an adapted 16-session diabetes prevention program (DPP Prevent+) based on the CDC’s National Diabetes Prevention Program PreventT2 curriculum. The program is designed to be delivered over approximately 6 months by trained lifestyle coaches and incorporates immunization education into selected sessions alongside the core diabetes prevention content as a complement to the program’s focus on prevention and health promotion. The inserts are designed for use by lifestyle coaches during select group sessions.

How It Works (Delivery Methods)

The inserts are designed for use by lifestyle coaches during select group sessions. They aim to gradually build awareness, confidence, and understanding about adult immunization within the broader context of diabetes prevention and health- promoting behaviors.
Each insert aligns with the topic of a specific session and introduces research-based, immunization talking points that connect to the session’s core content. The immunization content is intentionally designed to complement, rather than compete with, the program’s primary focus on diabetes prevention and sustainable lifestyle behavior change.

Expected Outcomes

Participants will:

• greater understanding of the role immunizations play in reducing complications from vaccine-preventable diseases, especially among people with or at risk for chronic conditions
• increased confidence in making informed choices about immunizations
